I did this trip in October 2010 and am pretty sure that I didn't take malarials. I just made sure I was well deet'd up when we went to Ranthambore.
Enjoy - it's a great, if very busy, trip - Varanasi is amazing!
None of the areas are particularly high risk though the evening and early morning game drives into the Ranthambore Tiger Reserve would definitely represent the highest risk of the trip. Cover up as much as possible during these two excursions with DEET based insect repellents applied in accordance with manufacturers instructions on exposed skin. This advice should be followed throughout the entire trip to prevent insect and mosquito bites that may transmit Malaria as well as other diseases like Dengue Fever and Chickungunya.
